Your First Visit
We want you to know this first and foremost about your first experience with us: Our priority is YOU – not your insurance, our policies, or our administrative logistics.
However, we don’t believe in surprises at Total Health Solutions. We will work out the financial arrangements that best fit your needs and your insurance coverage, if applicable, and discuss that with you prior to your visit.
All new patients meet with Doctor Mitch and receive a thorough history and chiropractic examination before receiving any chiropractic treatment. If x-rays, physical therapies, home therapies, nutritional therapies, etc. are recommended, the cost of such recommendations, and any other questions you may have, will be discussed with you prior to entering these recommendations.
Allow approximately forty-five minutes for your care on your first visit. We look forward to serving you in our clinic!

For your convenience, you may fill them out here and print them to bring to your first appointment. If, for some reason, you are not able to fill out and print these forms, we can give you the forms to fill out prior to your visit when you come in for your first appointment.
